How to Send Us Your Handpieces

Below are two items you will need to print out to get your handpiece to us. You may click on each item to open them off the site as a PDF, or you can "Right Click" and “Save As" to save them to your computer for printing. Both PDF's below are "fillable," meaning you can use your keyboard and mouse to fill in the information.

Step 1:

Print & Fill out the Work Order Form - Place Inside Box

You may click on each item to open on your screen, or you can "Right Click" and “Save As" in order to save to your computer for printing.

Please print this out to give us an idea of what is wrong; what you would like done; and if you prefer a free estimate beforehand.  We recommend keeping a copy for your records.

Request a Pre-Paid USPS Shipping Label from Us - call 315-689-6300 or email us below.
(We will send your Label via Email or Fax)

• Contact Person at your Practice
• Office name and address
• Telephone
• Fax – if you prefer the USPS label faxed to you
• Email – if you prefer the USPS label emailed to you
